Job Description

Caregiver (CNA) Job Description

Sustained Hope, LLC will only employ certified nurse aides (CNAs) to perform client’s personal care tasks.

Minimum Qualifications/requirements:

  • Must be 18 years of age.
  • GA-registered and GA-certified—state issued CNA card is required for verification. Completion of the National League for Nursing (NLN) exam online and competency assessment for PCA services is required.
  • At least one year of verifiable caregiving experience (previous work history in a PHCP organization is preferred).
  • Signed statement of no misconduct is required (with no recorded history of abuse/neglect of others) at the time of application.
  • Recent documentation of TB testing—skin and/or blood test (within last six months).
  • Valid GA driver’s license with a copy of DMV driving record.
  • Dependable and properly insured personal vehicle.
  • 3 references, with at least one from professional source.
  • Physically fit—able to walk, bend, stoop, turn, lift (up to 50 lbs.), etc.

Duties/Responsibilities:

  1. The Caregiver is responsible for the overall care for specifically assigned client(s).
  2. The Caregiver is responsible for learning, implementing and maintaining the core values of the Company.
  3. The Caregiver is responsible for delivering first class service of excellence to each and every client of the Company.
  4. The Caregiver is responsible for communicating all schedule changes, concerns, and any other care related issues directly to the Office/Care Manager for each client they have been assigned to.
  5. The Caregiver is responsible for documenting in accordance with the company documentation system set forth.
  6. The Caregiver is responsible for studying and carrying out each “Care Plan” in detail for each client they are assigned to.
  7. The Caregiver is responsible and accountable for accurately and honestly clocking-in and clocking-out using the Company Timekeeping System.
  8. The Caregiver fully complies with all reporting requirements, as given in the Company Policies and Procedures Manual.
  9. The Caregiver is expected to perform the duties that are outlined in each care plan. Caregiver understands that if there is any concern with any of the duties listed below, that he/she will immediately express the concern to a Supervisor who can assist with additional training on any of the duties/service items:
    1. Companionship
    2. Laundry
    3. Light housekeeping
    4. Meal planning and preparation
    5. Bathing/showering assistance
    6. Personal hygiene
    7. Errands & shopping
    8. Incidental transportation (preapproved)
    9. Medication reminding
    10. Incontinence related duties
    11. Additional duties requested by the client and approved by Sustained Hope

Companion/Sitter Job Description

Sustained Hope will employ qualified and trained individuals to provide client’s companionship/sitting services as well as to perform certain personal care tasks.

Minimum Qualifications:

  • Functionally literate (able to read and write and understand and follow written and verbal instructions; high school diploma preferred).
  • 18 years of age, with a valid GA license.
  • Physically able (lifting, stooping, turning, bending, squatting, kneeling; constant/repetitive standing; requires normal, or corrected vision and hearing, and the ability to accurately discern color as necessary to perform job functions).
  • Must clear background check (no misconduct/abuse) and TB-skin test.
  • Previous companion/sitting experience preferred.

Training & Assessment:
Sustained Hope will hire companions/sitters that have had some training in understanding the client needs and desired services that include home safety, light housekeeping, preapproved transportation services, handling at-home emergencies, infection control, and reporting requirements (incidents and complaints). Regardless of their prior training and experience, all Sustained Hope employees will receive general client-specific orientation before commencing their job.

  5. The Companion is expected to perform the duties that are outlined in the Care Plan. Companion understands that if there is any concern with any of the duties listed below, that he/she will immediately express the concern to a Supervisor who can assist with additional training on any of the duties/service items:
    1. Companionship (reading, talking, walking, watching TV, etc)
    2. Laundry
    3. Light housekeeping
    4. Meal planning and preparation
    5. Errands & shopping
    6. Incidental transportation (preapproved)
    7. Medication reminding
    8. Additional duties requested by the client and approved by Sustained Hope
